Dear <Name of your Creditor>,

This letter serves as a formal demand for you to immediately cease making unauthorized credit inquiries into my credit files.  Furthermore, I demand that you contact each CRA (Equifax, TransUnion and Experian) and request that they remove any and all of these “hard” credit inquiries against my credit.

 Here is the information required to locate the unauthorized credit inquiries:

 Insert your full name

Insert your complete address

Insert your SSN

Insert your Date of Birth

Insert date inquiry (s) appear in your credit file

Insert Consumer Credit Agency displaying the inquiry

 To the best of my knowledge, I have not expressly provided written authorization for your organization to view my credit history.  Therefore, you may have violated the Fair Credit Reporting Act, Section 1681©:  Transactions Not Initiated by Consumer.

 Please respectfully remove the aforementioned credit inquiry or provide me with documentation proving your authorization to access my credit file within 30 days.
